Will Medicaid Pay for Home Health Care for Seniors in Indiana?

As we age, many of us get to a point where we need assistance with daily tasks and/or we need someone to monitor health conditions. Understandably, most older adults would find themselves in need of assistance would prefer to remain in the comfort of their own homes rather than relocate to a nursing facility. Aging in place allows seniors to retain a sense of independence, continue familiar routines, and stay connected to loved ones. The costs associated with home-based care, however, can be significant, leaving many families uncertain about how to pay for the assistance their loved one needs. While Medicaid does cover long-term care expenses, the eligibility requirements and benefits, including the availability of home health care, vary from state to state. To help clarify your options, the Indianapolis attorneys at Frank & Kraft explain whether Medicaid coverage applies to home health services for seniors in Indiana.

Medicaid and Home-Based Care in Indiana

Medicaid is a joint federal and state initiative that provides health coverage for individuals with limited financial resources. In Indiana, the program is administered through the Family and Social Services Administration (FSSA). For seniors who require long-term care, Indiana Medicaid offers several programs that can help pay for in-home services, reducing the need for institutional placement.

One of the most important resources in Indiana is the Indiana Pathways for Aging Waiver, which enables eligible seniors to receive care at home or in community-based settings. This waiver is designed for individuals who meet the medical criteria for nursing home placement but wish to remain at home. Services available through the Pathways Waiver may include assistance with daily living tasks, skilled nursing visits, transportation, home-delivered meals, and therapies prescribed by a physician.

To qualify, applicants must meet both financial and medical requirements. From a clinical standpoint, the person must demonstrate a need for support equivalent to what would be provided in a nursing home. Financially, Medicaid imposes strict limits on both income and assets. As of 2025, a single applicant in Indiana must have income of less than $2,901 per month and countable resources of no more than $2,000. Certain assets, such as a primary residence, household goods, and a vehicle, may be exempt from this calculation.

What Services Does Indiana Medicaid Cover?

If approved, seniors can receive a wide variety of supports to help them remain at home safely. These services are typically delivered according to an individualized care plan developed by healthcare professionals, approved by Medicaid, and reassessed periodically to ensure it continues to meet the seniorโ€™s evolving medical and functional needs. Examples of what services may be covered include:

  • Personal care assistance such as bathing, dressing, grooming, and toileting.
  • Skilled nursing support, including medication management, injections, or wound care.
  • Rehabilitative services such as physical, speech, or occupational therapy when ordered by a physician.
  • Homemaker services, including light cleaning, laundry, meal preparation, and shopping.
  • Respite care for family members who provide day-to-day support, giving them temporary relief.

Paying Family Members as Caregivers

Many families ask whether Indiana Medicaid allows relatives to be paid for providing care. Under certain circumstances, this is possible. Through the Pathways Waiver and Indianaโ€™s self-directed care options, seniors may choose their own caregivers, which can include a trusted family member or friend. In these situations, the senior acts as the employer, while a fiscal intermediary helps with payroll and compliance with program rules.

That said, there are restrictions. Spouses and legal guardians are generally not eligible to receive payment due to potential conflicts of interest. The family member providing care may need to complete training or certification recognized by the state, such as Home Health Aide or Personal Care Assistant qualifications. Payment is subject to limits, including caps on the number of reimbursable hours and rates that are set by Medicaid and usually lower than private-market compensation. Finally, the arrangement must be incorporated into the seniorโ€™s approved care plan and overseen by a case manager to ensure it aligns with the recipientโ€™s needs.

The Importance of Planning Ahead

Applying for Medicaid in Indiana can be a complicated process because income and asset rules are strict, and mistakes can delay approval or result in denial. For example, transferring property or money shortly before applying may trigger a penalty period, during which Medicaid will not pay for services. Incorporating a Medicaid planning component into your comprehensive estate plan is the best way to protect assets while ensuring that eligibility requirements will be met when care is needed.
